When To Combine a Lift With Breast Implants
Breast augmentation makes the breasts fuller but does address sagging and loose skin. Breast implants alone may be enough to correct mild sagging (ptosis), but a breast lift is needed to raise and reshape the breasts when the nipples rest below the breast crease or point downward.

Customizing Your Breast Augmentation
You have several choices regarding your breast augmentation procedure. Dr. Parker clearly explains these options during your consultation and uses his expertise to help guide your decision-making process while ensuring your aesthetic goals remain at the forefront.
Your choices include the type, size, and profile of the implants, the incision location, and where Dr. Parker places the implants.
Breast Implant Type: Silicone Gel or Saline?
One of the first choices breast augmentation patients make about their implants is whether to get silicone or saline. It is largely a personal decision, and a skilled plastic surgeon such as Dr. Parker can achieve very natural-looking results with either option. At your consultation, Dr. Parker will discuss the differences between each breast implant type and give you samples to hold in your hands so you can feel the differences for yourself.
Silicone gel implants feel the most like natural breast tissue. They come pre-filled to the correct volume with a cohesive gel material. In accordance with FDA guidelines, silicone implants are available for women aged 22 and older or patients getting a breast lift.
Saline implants have a silicone outer shell filled with a sterile saline solution after placement within the breast to achieve your desired breast size. For women aged 18 to 21, saline implants are the only option unless they are having a breast lift.
You can read more about the different types of implants in one of Dr. Parker’s blog posts. Whether you choose silicone gel or saline, Dr. Parker uses smooth, round implants because he believes they provide the softest and most natural-looking breast augmentation results.
Our Experienced surgeon
Dr. Paul M. Parker
Board-certified plastic surgeon Dr. Paul M. Parker combines an unwavering dedication to patient satisfaction with surgical expertise to create aesthetically pleasing, natural-looking results.
Most patients arrive at their consultation with an ideal bra cup size in mind for their results. That gives Dr. Parker a general sense of your aesthetic goals, but it’s important to understand that implant sizes (measured in cubic centimeters or cc) don’t translate to a certain cup size. Getting 400 cc implants, for example, won’t necessarily create D-cup breasts for every patient.
The best way to choose the size of your implants is to have your implants sized to your body and your desired outcome. At the Parker Center, we schedule a custom Sizing and Shaping session, where you can try different implant shapes and sizes based on those that will best fit your frame. During the session, you’ll also get to see realistic 3D simulations of your body using the Vectra® 3D imaging system, showing different sizes of implants so you can compare your options and see which look you like best.
Which Incision Location Is Best for Breast Augmentation?
During breast augmentation surgery, Dr. Parker inserts implants through small incisions he makes in one of 3 locations. These locations include in the crease under the breast (inframammary), around the lower half of the areola (periareolar), or in the armpit (transaxillary). He discusses the options with you and considers several factors when choosing which option is best for you. These factors include:
Which location do you feel is least noticeable?
Which incision enables Dr. Parker to best meet your goals?
Are you considering breastfeeding in the future?
Which type of implant are you getting?
The most common incision location is under the breast, but you make the choice with Dr. Parker’s guidance. Regardless of the incision location, the resulting scar following Dr. Parker’s careful closure technique usually flattens, fades, and becomes inconspicuous over the next several months.
What Is the Best Placement for Breast Implants?
Even though you may have read that you have 2 options for implant placement—over or under the chest muscle—there’s a little more to it than that. Dr. Parker customizes implant placement based on your needs.
Dr. Parker often uses a technique called dual plane breast augmentation, which involves placing the implant partially behind the muscle. Using this technique, Dr. Parker can place the implant with more precision and create a beautiful breast shape that looks natural. He also believes patients are less likely to develop a complication called capsular contracture with dual plane placement and that radiologists see breast tissue more easily on mammograms.
What Is a Breast Implant Profile?
The profile of an implant refers to its height and how far it projects from your chest. Dr. Parker explains the differences in detail during the consultation. Your anatomy and the size of the implants you want determine the best implant profile for you.
Thinner patients or those with a smaller frame and narrow chest wall typically do best with high-profile implants, which have a greater forward projection to achieve the desired size increase with a smaller base width. Those with a wider chest wall tend to do well with moderate plus-profile implants.
Which Implant Brand Should I Choose?
Dr. Parker has used Mentor® breast implants almost exclusively for much of his career. Here’s why he believes them to be a superior product for his patients:
Mentor implants have a lower deflation rate, which means that our patients are less likely to need secondary surgery due to deflation.
In our experience, the outer shell of the Mentor implant is less likely to wrinkle when implanted.
Again, due to differences in the shell, we find that Mentor implants are less palpable (less able to feel the edges) beneath the breast.
What Happens During Breast Augmentation Surgery?
Dr. Parker performs breast augmentation surgery in our fully certified, onsite, ambulatory surgical center. A board-certified anesthesiologist places you under general anesthesia so you’ll be asleep throughout the procedure.
Breast augmentation surgery typically takes 1 hour or more, depending on the specific details of your procedure. For example, if you’re combining breast augmentation with a breast lift or if Dr. Parker needs to use different sizes of implants to address asymmetry, the procedure may take longer.
During the procedure, the anesthesiologist will raise up the back of the operating table several times, allowing Dr. Parker to evaluate your breasts in this “beach chair” position for precise size and shaping. He never rushes, and the procedure is not finished until everything looks perfect to him. You’ll return home after spending a brief time in the recovery room.
Your Breast Augmentation Consultation & Imaging Session
Consultations at the Parker Center are special. Dr. Parker will spend extra time with you, ensuring that he understands your goals and that you are fully educated about each potential solution. Additionally, our imaging expert will help you see the possibilities with digital photo imaging to show you what you will look like after surgery. Patients who are comparing providers often tell us our consults are the most thorough and informative they’ve ever experienced!
One to two weeks before your scheduled breast augmentation surgery, you will undergo a one-hour, in-office, face-to-face education session with one of Dr. Parker’s Nurse Educators. During this session, the nurse educator thoroughly reviews a detailed information packet to further educate you about your breast augmentation. This includes a discussion of what to do to prepare for surgery, what occurs on the day of surgery, and what to expect after surgery during your recovery. It’s a detailed session that really educates our patients about the entire surgical process at the Parker Center. Patients tell us they love it because they feel so well-prepared for their surgery. Learn more about what to expect during your pre-op education session.
Rapid Recovery Breast Augmentation
The Parker Center’s Rapid Recovery techniques used during and after your breast augmentation help you heal more rapidly and comfortably than what you might experience at another practice. This means less time off from work and a sooner return to your normal activities.
Dr. Parker’s Rapid Recovery protocol includes:
Short-acting anesthetic agents and anti-emetic medications to hasten the recovery from anesthesia
A tailored regimen of medications given before, during, and after surgery to minimize postoperative pain, swelling, and bruising
Three postoperative medical manual lymphatic drainage massages to reduce swelling and bruising as well as accelerate healing
Topically applied state-of-the-art scar cream to help minimize the scars’ appearance.
Even though everyone heals at their own pace, our breast augmentation patients are usually back to work within the first week if they have non-physical jobs. We encourage you to move around briefly starting the first day to boost circulation.
All sutures used are dissolvable, so you won’t need to come to the office to have them removed. Pain is rarely an issue following breast surgery at the Parker Center, but you can expect some post-operating swelling and soreness. Some patients even feel good enough to go to dinner the day after surgery, while others prefer waiting a few days before socializing.
Our Signature
Rapid Recovery Program
Our surgical procedures, anesthesia, medications, and aftercare are tailored to minimize post-operative discomfort and accelerate recuperation.
Can I still breastfeed after getting breast implants?
Dr. Parker will be careful to preserve the milk ducts and glands when performing your procedure to avoid interfering with your ability to breastfeed. It’s important to let your surgeon know about your future plans during your consultation.
Is there anything I should avoid after surgery?
To steer clear of complications and optimize your results, avoid the following after breast augmentation: Sleeping on your stomach: It’s best to sleep propped up with pillows or in a recliner for the first few days after surgery to minimize swelling and keep pressure off your breasts. Wearing underwire bras: Initially, you will need to wear a soft, supportive bra. Underwires can put stress on the incisions and reduce blood flow while they are healing. Swimming: To avoid infection, allow the incisions to heal properly before taking a dip in a swimming pool or the ocean. Exercising strenuously: While gentle walking is encouraged after surgery, postpone vigorous activities for at least 6 weeks. Drinking alcohol and smoking: Focus on your health during your recovery period to help—not hinder—the healing process. Contact our office if you have any questions about these restrictions, or ask Dr. Parker during a follow-up appointment.
How long does it take for implants to settle?
It usually takes about 6 to 8 weeks for the breasts to soften and implants to settle into a natural-looking position as tissues relax and swelling subsides.
Is breast augmentation surgery safe?
When performed in an accredited surgery center by an experienced, board-certified plastic surgeon using a board-certified anesthesiologist, breast augmentation is very safe for properly screened patients.
