Case Details
This 52 year old requested improvement with facial aging. She was bothered by excess upper eyelid skin, puffiness of her lower eyelids, and felt her face had generally lost its youthful shape. The patient previously had Botox injections to her glabella and forehead as well as hyaluronic acid fillers to her nasolabial folds and pre-jowl areas. She wanted to avoid having Botox injections in future, if possible.
Dr. Parker discussed various options with the patient during her consultation. He explained how he could perform an upper eyelid lift to remove excess eyelid skin. In addition, through the same incisions, several other things could be accomplished. A portion of her corrugator muscles could be removed to reduce wrinkling between the eyebrows and forehead and thus reduced the need for future Botox. In addition, a brow lift could be performed to favorably reposition her brows. Lastly, Dr. Parker explained how her lower eyelid concerns could be addressed by removing then repositioning fat into her tear troughs, excising loose lower eyelid skin, and finally tightening the remaining skin with a TCA peel.
He supplemented his discussion by showing her examples of similar patients as well as 3-dimensional computer imaging to allow her to see what the changes he suggested could do for her. The patient and Dr. Parker agreed upon a comprehensive plan which was carried out in the operating room several weeks later:
1. Upper eyelid lift with excision of her corrugator muscles and a lateral brow lift.
2. Lower eyelid lift with removal and transfer of fat to her tear troughs, lateral canthopexy, excision of excess skin and a TCA peel to treat fine wrinkles.
3. Fat injections to her cheek bones, nasolabial folds and pre-jowl areas.
The patient said she experienced no pain after surgery. Once bruising and swelling subsided she returned to her normal activities. She is very happy with the natural rejuvenation of her facial features and eyelids, remarking that she “looks less tired, more well rested” and that her eyes appear “brighter and more open.”
