Breast Implant Revision Before & After Case 1220

This 43 year old patient was referred to Dr. Parker by another patient requesting improvement in the appearance of her breasts. She said she had 500 cc saline implants inserted into a dual plane location through transaxillary incisions elsewhere by another plastic surgeon 20 years ago. Over time the right implant has drifted inferiorly and laterally so it is under her right armpit. More recently, the left implant has progressively deflated creating a significant asymmetry. The patient told Dr. Parker her goals were to downsize her implants, convert from saline to silicone, move the implants out from under her armpits and create more cleavage. The patient is 5’4” tall and weighs 125 pounds. She and Dr. Parker then arrived at the following plan which was subsequently carried out through inframammary incisions: the existing 500 cc saline implants were removed, the contracted left pocket expanded medially to create more cleavage, the over expanded right pocket reduced in size with thermal and suture capsulorraphies as well as the insertion of Galaflex absorbable mesh, 375 cc silicone implants were then inserted. The patient was delighted with her quick recovery and the markedly improved appearance of her breasts
