Case Details
This 35 year old came to Dr. Parker requesting rejuvenation of her lower eyelids. She was happy with the appearance of her upper eyelids. The patient expressed the following to Dr. Parker: she was constantly told she looked tired and did not like the puffiness of her lower eyelids, or the depression just beneath the puffy area. Dr. Parker examined her and explained that the puffiness of her lower eyelids was caused by fat which normally resides around the eyeball herniating forward and pressing outward against the skin. The depression beneath the lower lids is called the “tear trough” and is a result of ligaments which are firmly adhered to the underlying bone. In combination, these factors were causing the patient to look tired. Dr. Parker went on to say that some of her herniated fat could be removed through an incision on the inner aspect of the lower eyelid. This is called a transconjuntival blepharoplasty. The remainder of the fat could be transferred with sutures to fill the tear trough. Finally, her lower eyelid skin would be tightened with a trichloroacetic peel. The patient liked that there wouldn’t be any visible incisions or scarring. Dr. Parker subsequently performed the procedure and the patient had a normal recovery. She has been told how refreshed and well-rested she now looks, boosting her self confidence.
